Summary:
The Caring Connection is hiring for caring and compassionate Personal Care Attendants/Caregivers with or without a CNA license to work with patients throughout Los Angeles County. The Personal Care Attendant/Caregiver will be responsible for assisting patients with their personal care and hygiene needs, all activities of daily living and instrumental activities of daily living (ADLs/IADLs), accompanying patients to appointments and on errands as needed, health monitoring, and helping to maintain a clean and safe home environment for each patient. The successful Personal Care Attendant/Caregiver will also be required to maintain accurate and current health records on each patient.

Registered Nurse (RN) Responsibilities:
- Administer medications and render treatment to home health patients under physician's orders
- Communicate with physicians, other RNs, LVNs, pharmacists, administrative staff, and other healthcare professionals
- Collaborate with physicians to create a Plan of Care for each home health patient
- Create and contribute to the patients' medical records as needed
- Respond to injuries, concerns, and complaints from home health patients and their families
- Track and maintain medical supply inventory along with other staff members
- Counsel patients and family members before, during, and after treatment
- Facilitate and coordinate care among various disciplines including LVNs, PT/OT/ST/MSW, etc. as needed
- Complete and submit required documentation according to Agency policies and procedures
- Other duties as assigned
Registered Nurse (RN) Qualifications:
- Valid CA Registered Nursing license
- Advanced knowledge of EMRs
- 1+ year(s) of nursing experience, preferably in home health
- Willingness to work a flexible schedule and fill-in when needed
- Pediatric care experience a plus
- Bilingual Spanish is desirable

- Provide wound care
- Help educate family and Caregivers / Home Health Aides as necessary
- Complete and submit required documentation according to Agency policies and procedures

- Current California LVN/LPN License

- Help with patient meal preparation and feeding, as well as basic housekeeping functions
- Assist
with all activities of daily living and instrumental activities of
daily living according to patient's personalized care plan
- Work
with transportation professionals to make sure that patients requiring
additional assistance getting in and our of vehicles are able to safely
use transportation as needed
- Deliver weekly reports to office staff and review important changes in the patient's condition
- Other duties as assigned
Personal Care Attendant (PCA/CNA) Qualifications:
- 1+ year(s) experience as a Personal Care Attendant/Certified Nursing Assistant
- Bilingual Spanish a plus
- Prior pediatric experience and/or prior experience with developmentally disabled individuals desireable
- Ability to work a flexible schedule (Weekends, holidays, and evenings in high demand)
- Must be able to pass a comprehensive criminal background check, be willing to submit to random drug screening, and be legally authorized to work in the United States
