Home Health Care

Home health consists of services including nursing, PT/OT/ST, medical social services, certified home health aides, and personal care services, administered in the patient's home by professional care providers who are specially trained and experienced in home health. These services may be intermittent visits or extended continuous care.

Skilled Nursing

Nurses evaluate and assess patient medical needs and render treatment, instruction and teaching in accordance with the plan of care, which is duly approved by the attending physician. Services which  nurses may provide in the home include:

Social Services

Medical Social Workers will be utilized to assist you and your family with the physical and emotional issues imposed by illness. They are also trained to assist you with identification and utilization of additional community resources.

Occupational, Physical, and Speech Therapy

Certified therapists are available to assist the patient in improving the skills needed to perform daily activities.

Home Health Aides

Certified Home Health Aides will assist you with your activities of daily living including, but not limited to, grooming, bathing, hygiene, meal preparation, and light housekeeping. CHHAs can also provide companionship, safety supervision, and transportation to appointments and activities.
